This programme is intended for Transition Year students ONLY.

I do hereby give RCSI University of Medicine and Health Sciences the right to use my child’s name, picture, photograph, image of them, in all forms of RCSI approved publications (e.g. annual reports, university newsletters and magazines), media materials (e.g. newspapers, TV and online media), promotional materials (e.g. outdoor advertising, brochures, prospectuses, posters, banners, online advertising), digital platforms (RCSI websites) and social media platforms, and I waive any right to inspect or approve the finished product, including written copy, that may be created in connection therewith. I also agree that this releases RCSI and its representatives from any and all monetary obligations or payments to me and my child or any or all of our authorised representatives for use of photographs and/or image of my child either now or at any time in the future. Whilst RCSI will only circulate image footage to media consistent with RCSI’s values, it has no control over such images appearing on other social media and/or websites inconsistent with RCSI’s values. RCSI has no control over images/ videos taken by other participants on the programme and is not responsible for any third-party images/ videos taken or shared.

School Permission: By completing this form, you are confirming that you child's school agrees to indemnify RCSI in respect of claims arising in connection with the RCSI’s TY MiniHealthScience Programme, where the participating school have been negligent. RCSI accept no legal liability for bodily injury or property damage, for accidents arising in connection with students travelling to or from the RCSI TY MiniHealthSciences Programme.

Parent/Guardian: I have read and explained this complete code of conduct to the registered student who agrees to abide by its terms. I agree to permit students to attend RCSI’s TY MiniMed / TYMiniDentistry/ TYMiniPharm/ MiniFutureScientist/TYMiniPhysio online programme. I understand that they will be entering (via onscreen format) medical environments and the hazards that this might entail. I understand that they have a responsibility to behave in an appropriate manner in these environments and if they do not they will not be permitted to attend the rest of the programme.
